Fall sports are almost here! If you're a District 186 middle or high school student planning to participate in athletics this school year, now is the time to schedule your IHSA sports physical.
Memorial Health is offering 3 opportunities for our athletes to get their sports physicals!
-Schedule your Appointment today (Appts cannot be changed once scheduled)
- Bring a copy of your insurance or medical card. No payment is due at the appointment. Insurance will be billed.
-Please arrive at least 10 minutes before your scheduled appointment.
